Nicole Falanga, M.D.

Born in Warwick, New York, I spent my early years immersed in art, pottery, music, and dance, encouraged by my mother to explore many forms of creative expression. I attended and graduated from the Waldorf School in Chestnut Ridge, New York — an education that was academically rigorous while also deeply nourishing to the spirit through music, art, movement, and literature. Science and mathematics were always among my favorite subjects growing up, and the Waldorf approach beautifully balanced these analytical interests with creativity, imagination, and reverence for the human experience.

Baptized in the Catholic Church as an infant, and during my early childhood, I was profoundly influenced by Gurumayi Chidvilasananda, the spiritual head of Siddha Yoga in South Fallsburg, New York. I still carry peaceful memories from 1989, when I was nine years old — the meditation caves, the resonance of collective chanting that could be felt within the chest, the fragrance of incense, and the deep sense of devotion and service through Seva. I cherished small sacred gifts from that time, including a red velvet cloth from Muktananda’s pillow and gold butterfly earrings gifted to me by Gurumayi, who gave me the Sanskrit name “Pushpa,” meaning “the blossoming of inner devotion.”

Toward the end of adolescence, I explored the teachings and group discussions of the Gurdjieff movement before eventually entering medical school. When it came time to choose a specialty, Emergency Medicine felt like the only path aligned with who I was at that stage of life. I was fortunate to complete my Emergency Medicine training in New York City, serving communities in East Harlem and West Harlem. The work was demanding, humbling, and deeply rewarding. During those years, I witnessed both the fragility and resilience of human life on a daily basis.

After more than a decade of education and training devoted to saving lives and treating acute illness, I found myself increasingly drawn toward preventative and integrative approaches to health. I often wondered: what might our Emergency Departments look like if more time, energy, and resources were devoted to cultivating wellness before disease emerges?

My professional evolution was shaped by my own desire to become physically stronger, healthier, and more grounded. Through Yoga Teacher Training, I deepened my understanding of the body, breath, and nervous system. My initiation into Reiki further expanded my perspective on healing and reinforced my commitment to practicing medicine that honors the whole person — mind, body, and spirit.

In 2023, I officially opened Chestnut Ridge Wellness Medicine, my home-based integrative medical practice. My work blends the foundation of evidence-based Western medicine with complementary healing modalities, emphasizing prevention, resilience, and whole-person care. I believe healing occurs not only through medications and procedures, but also through connection, movement, nutrition, rest, emotional well-being, spiritual inquiry, and the restoration of balance within daily life.

In my free time, I enjoy traveling - Bali was my most recent trip which was transformative - gardening with my husband, reading psychology — likely inspired by being raised by a hypnotherpist and clinical social worker — as well as philosophy and world religion texts, deepening my understanding of healing modalities and medicine, practicing yoga asana, sharing meals and meaningful connection with friends, and raising my daughter and son with my husband.

Foundational Wellness Visit

The Foundational Wellness Visit is a comprehensive medical visit designed to establish a clear understanding of your health concerns, lifestyle, and overall well-being. This is the cornerstone visit. The visit includes a Life Biography overview, exploration of the primary concern that brought you to care, and simple centering techniques to support presence, self-awareness, and meaningful engagement in the healing process. This visit is a thorough review of your past medical history, laboratory studies, imaging, medications, nutrition, sleep, stress, movement, social factors and is combined with a physical examination. Recommendations may include additional laboratory or radiologic evaluation, conventional medical treatment, lifestyle interventions, nutritional support, homeopathic remedies, or other integrative therapies as clinically appropriate. 

This visit serves as the foundation for an individualized treatment plan and is followed by an Integrative Wellness Visit to review findings, assess progress, and refine recommendations. (60-90 Minutes) Self-pay $525-$795. Wellness Visits are billed using the appropriate ICD and CPT medical codes and may be submitted for reimbursement to your health insurance company.

Integrative Wellness Visit

The Integrative Wellness Visit is a comprehensive follow-up consultation designed to build upon the foundation established during your initial evaluation. Together, we review your primary health concerns, progress since the Foundational Wellness Visit, laboratory and imaging results, and any treatments, remedies, lifestyle changes, or therapeutic practices that have been implemented.

This visit provides an opportunity to assess what has been helpful, identify ongoing challenges, and refine your personalized treatment plan. Additional laboratory evaluation, diagnostic testing, or treatment recommendations may be considered when clinically appropriate. Integrating conventional medical care with evidence-based lifestyle and complementary approaches, the goal is to support sustainable improvements in health, resilience, and overall well-being.

Visits are individualized (60–90 Minutes) Self-pay $350–$495. Wellness Visits are billed using the appropriate ICD and CPT medical codes and may be submitted for reimbursement to your health insurance company.

Medical Biography Visit

Health concerns rarely arise in isolation. A Medical Biography Visit offers an opportunity to explore the story behind a symptom, illness, or health challenge through a comprehensive, physician-guided conversation. Drawing upon principles of lifestyle medicine, mind-body medicine, and narrative inquiry, this visit examines how life experiences, relationships, stress, habits, beliefs, and personal history may influence physical and emotional well-being. By understanding the broader context of a health concern—whether related to sleep, anxiety, weight, digestion, chronic illness, or other symptoms—patients gain deeper insight into contributing factors and pathways toward meaningful healing. 

Medical Visits:  (60-90 Minutes) Self-pay $325–$395. Wellness Visits are billed using the appropriate ICD and CPT medical codes and may be submitted for full reimbursement to your health insurance company.

Acute & Chronic Unwellness Visit

The Acute & Chronic Unwellness Visit is designed for patients experiencing a sudden change in health, new symptoms, worsening of an existing condition, or ongoing health concerns that require timely medical evaluation. Whether addressing an acute illness, flare of a chronic condition, unexplained symptoms, or a setback in overall well-being, this visit provides a focused assessment to determine the most appropriate next steps.

Care may include a focused medical history, physical examination, review of medications and lifestyle factors, laboratory or imaging recommendations, and treatment options utilizing both conventional and integrative approaches when appropriate. The goal is to identify contributing factors, relieve symptoms, and support recovery while addressing underlying causes whenever possible.

Visit length is tailored to the complexity of the concern. Focused visits for straightforward issues may be completed in as little as 15 minutes, while more complex evaluations involving multiple symptoms, chronic conditions, or extensive review of medical history may require 60–90 minutes. Fees are structured accordingly to reflect the time and level of care provided.

 (30-60 Minutes) Self-pay $150–$295 Wellness Visits are billed using the appropriate ICD and CPT medical codes and may be submitted for reimbursement to your health insurance company.

   My pricing model reflects the depth, breadth, and integration of my training and experience. As a Western-trained physician who is Board Certified in Emergency Medicine, I bring a high level of clinical expertise, diagnostic precision, and experience managing complex and acute medical conditions. This foundation ensures that every patient encounter is grounded in strong medical judgment and evidence-based standards of care.

Appointments are intentionally longer and more comprehensive than traditional primary care visits, allowing for thoughtful dialogue, individualized treatment planning, and true partnership. The fee structure reflects this dedicated time, advanced training, and the personalized, integrative care model offered within the practice.
